Offering competitive wages and impressive benefits, Southeast Health Group is a nationally-recognized agency providing community-based health care services. Associated with a newly created program in Prowers County, HEALTH NAVIGATORS will coordinate care and provide information-tailored to the needs of each patient-while providing motivation and support. The purpose of the Navigation program is to increase patients' access to primary/behavioral health care, preventative care, and early intervention services, while offering team-based education and coaching with an emphasis on overall health. This is a full-time benefit-eligible position-based in Lamar-with a starting annualized salary of $30,000. Requirements:
